It replaces the Medical Council of India (MCI) with a National\nMedical Commission (NMC).<\/strong><\/li>\n\n\n\n<li>The National Medical Commission will comprise of\n25 members who will be appointed by the central government through a research\ncommittee. <strong>The fees of 40% of seats in private medical institutions will be\ndecided by the commission.<\/strong><\/li>\n\n\n\n<li>The bill will set up a Medical Advisory Council\nwhich will function as a medium to convey dialogue between the National Medical\nCommission and states\/union territories.<\/li>\n\n\n\n<li>Four autonomous boards will be established under\nthe commission to supervise and assess the undergraduate and postgraduate\nmedical education.<\/li>\n\n\n\n<li>The National Medical Commission will regulate\nmedical education as well as a medical practice.<\/li>\n\n\n\n<li>A National Licentiate Examination will be\nconducted to get the license for medical practice as well as to get admission\ninto the postgraduate medical course.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Examinations: <\/strong><\/h3>\n\n\n\n<ul>\n<li>The bill provides for a consolidated NEET\n(National Eligibility-cum-Entrance Test) for admissions in all undergraduate\ncourses by which separate exams and multiple counseling processes for various\nmedical colleges will be avoided.<\/li>\n\n\n\n<li>For admission in postgraduate level, an\nequivalent NEXT will be conducted. It will also be the Licentiate Exam for\nobtaining a license to practice.<\/li>\n\n\n\n<li>This provision will erase off the multiplicities\nof exams and abolish the inequality in the skill set of doctors graduating from\nall parts of the country.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>License:<\/strong><\/h3>\n\n\n\n<ul>\n<li>The doctors will need to qualify the Licentiate\nexam only once in the lifetime. Considering the continuous developments in\nmedical science and technology, this license system keeps no checks if a\ndoctor\u2019s knowledge and skills are up to date with the changing times.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Fee Structure:<\/strong><\/h3>\n\n\n\n<ul>\n<li>The old National Medical Council Act has no\nprovision to regulate fees of various medical institutions. The IMC bill offers\nto cap fee by enabling the commission to frame guideline for profit-driven\nprivate institutions. <\/li>\n\n\n\n<li>The IMC can directly determine the fee for up to\n50% of the seats in private medical colleges. This will provide impetus to\ntalented and meritious students to avail the education opportunity in best\ninstitutions.<\/li>\n\n\n\n<li>On one hand, it is a positive step in the\ndirection of \u2018free education for all\u2019, on the other hand, this provision can be\nproved to be as a discouraging step for private investments in the field of medical\neducation. Thus there is a need to incentivize private entry by reducing\nbarriers to open medical colleges.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Representation in the NMC:<\/strong><\/h3>\n\n\n\n<ul>\n<li>A search committee will recommend the names of\nchairman and members of the National Medical Commission to the government.\nTwo-third of the members of the commission, including the chairperson, will be\nmedical practitioners. This could lead to too much influence of medical\npractitioners in the field, according to expert committees.&nbsp; <\/li>\n\n\n\n<li>However, the act asserts a transparent process\nfor selecting the members. The NMC, as well as the search committee for NMC,\nwill be a mix of nominated as well as elected members.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Autonomy of the Commission<\/strong><\/h3>\n\n\n\n<ul>\n<li>The present framework of the bill provides\ncomplete autonomy to the National Medical Commission. However we must not\nforget that the government should have an overriding supervisory power over the\nNational Medical commission so that the regulations formulated by the\ncommission concur with the government policies. <\/li>\n\n\n\n<li>Moreover, the government is expected to address\npublic emergencies. It might not be the best choice for the commission to\ndispense government\u2019s duties in emergency cases.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>How will it affect AYUSH?<\/strong><\/h3>\n\n\n\n<ul>\n<li>The bill offers a bridge to allow AYUSH workers\nto practice allopathic medicines. Now 3.5 Lakh AYUSH practitioners across India\ncan also add up to the already 11 Lakh registered allopathic practitioners.\nThis provision has both positive and negative sides.&nbsp; <\/li>\n\n\n\n<li>On one hand it can provide an integration of\nthese two medical schools, on the other hand, the AYUSH practitioners are\nalready well qualified so further integration will lead to overqualification of\npractitioners. At the same time, this step can also prove to hinder the\nundisturbed growth of AYUSH.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Disparities in healthcare scenario:<\/strong><\/h3>\n\n\n\n<ul>\n<li>The biggest challenge to the Indian healthcare\nsystem in present times is the unequal distribution of doctors between metro\ncities and rural areas. The current ratio between the number of doctors in\nmetropolitan and rural areas is 3.8 to 1. At a time when more than 70 percent\nof Indian population lives in villages, such disparity is frightening.&nbsp; The reason behind this discrepancy is\nInadequate investment. Furthermore, very few incentive structures in the rural\narea<\/li>\n\n\n\n<li>The qualification of doctors in the rural area\nis much lower than those in urban. The requirement in public health in rural\nareas differs vastly from what is being taught in MBBS schools. <\/li>\n\n\n\n<li>The disparity is also wide in the number of\ndoctors in northern and southern states. Southern states like Kerala and Tamil\nNadu have adequate doctors while the states like Bihar and Uttar Pradesh has an\nacute shortage of doctors.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Conclusion:<\/strong><\/h3>\n\n\n\n<ul>\n<li>We are facing an overall shortage of qualified\ndoctors in our country. According to the World Health Organization, the\nstandard population to doctor ratio is 1000:1. In India, the ratio stands at\n1456:1. Over 57 percent of the practicing doctors in India are unqualified with\na large number of unaccounted quacks in the field.<\/li>\n\n\n\n<li>While the National Medical Commission Act 2019\nhas brought some critical attention to it, the act brings a paradigm shift in\nthe philosophy of regulation. The number of doctors and their quality is bound\nto increase with the implementation of this act.
